When does empathy kick in?

For most people, empathy is something that happens naturally.  An example of “empathy kicks in” is when you are approaching a door and you see someone else approaching the same door with their hands full of something that they are carrying.  Empathy kicks in, and you imagine how hard it will be for them to open the door carrying so much.  So you offer to hold the door open for them.  Some might say that is being polite and while that is true, it is empathy that kicks in and causes your brain to react.  Empathy kicks in when you imagine and understand what someone else is feeling which causes your brain to react with a feeling, that causes you to do something.
